## What sits behind a sentence

Every clinical claim in the course carries a footnote to a primary document — a
consensus report, a regulator's publication, or a named journal article with its
DOI or PMID — recorded in a source registry held in the repository rather than on
a bibliography page. Practice items cite the same registry, so an explanation can
be traced to the document it came from.

Where a number could not be confirmed at the issuing body, the course says so and
no item is keyed on it. A confidently wrong threshold is worse than an
acknowledged gap.

## What the review binds

A credentialed dentist approves a file, not a product. The approval records a
SHA-256 digest of that file's exact text, recomputed on every read. If a single
character changes afterwards the digest stops matching and the file leaves the
product until it is reviewed again.

This fails closed: unreviewed clinical content is not shown with a caveat, it is
not shown at all. Re-stamping a digest after an edit would assert that the
reviewer approved text she never read, so the tooling refuses to do it.

## Where this is incomplete

The question bank does not yet cover the exam in proportion. The coverage matrix
in the repository reports items per Foundation Knowledge area and per Clinical
Content section against the official weights, including areas with none. It is
generated from the corpus and checked automatically.
