Most US states require graduation from a CODA-accredited dental hygiene program, but a few let a foreign-trained dentist qualify for a dental hygiene license with their dental degree. Florida (s. 466.007, F.S.) accepts graduates of unaccredited dental schools who show four academic years of dental education and a DDS-comparable diploma through an ECE year-by-year evaluation, submit a passed NBDHE or National Board Dental Examination with the credentials application, and then pass the ADEX dental hygiene exam and the Florida laws and rules exam. Indiana opened a route on July 1, 2026, and Virginia's law took effect July 1, 2026, but the Virginia Board says no licenses can issue until its regulations take effect, estimated for late winter or early spring 2027. Connecticut accepts a foreign DDS only with exams plus a year of clinical training through a Connecticut hygiene program. North Carolina, Georgia, Texas, and California do not offer the route. The NBDHE costs $600, the ADEX dental hygiene exam $1,150 plus facility and typodont fees, and the BLS median wage for dental hygienists was $98,100 in May 2025.

## Key facts

- **Open routes:** Florida · Indiana — Virginia's law is in force but licenses wait for regulations (estimated early 2027). Connecticut adds a year of training.
- **Written exam:** NBDHE, $600 — 350 items; a scale score of 75 passes. Dentists qualify if they meet INBDE eligibility. Florida also accepts the National Board Dental Examination.
- **Clinical exam:** ADEX hygiene, $1,150 — Plus a $150 typodont fee and a facility fee. International graduates need a state board authorization letter.
- **Median pay:** $98,100 — BLS median annual wage for dental hygienists, May 2025. Dental assistants: $48,070.
- **Assistant route:** DANB CDA Pathway III — Open to graduates of dental degree programs outside the US or Canada.
- **Not allowed:** NC · GA · TX · CA — North Carolina lets foreign-trained dentists work as a Dental Assistant II instead.

## Why foreign-trained dentists consider hygiene

The DDS route takes years: an [advanced-standing program](https://dentovio.com/international-dentist-programs/index.html.md) runs 24 to 45 months and often costs $300,000 or more. A hygiene license, where your state allows it, can put you back in clinical practice far sooner, earning a median of **$98,100** a year (BLS, May 2025), while you prepare for the dentist route or decide whether you need it.

Two caveats before you plan around it:

- **It is a different license.** A dental hygiene license does not let you practice dentistry, and the scope of hygiene practice is set by each state.
- **Few states offer the route.** JCNDE notes that every state accepts graduation from a CODA-accredited hygiene program. A foreign dental degree qualifies only where state law says so, which few states do.

## Which states let a foreign-trained dentist become a hygienist

| State | Status, November 2026 | What the law requires |
| --- | --- | --- |
| **Florida** | Open | Graduate of an accredited or unaccredited dental school; 4 academic years of dental education and a diploma comparable to a DDS or DMD, shown through an ECE year-by-year report; an HIV/AIDS course; NBDHE or National Board Dental Examination; ADEX dental hygiene exam; Florida laws and rules exam (s. 466.007, F.S.; rule 64B5-2.0144) |
| **Indiana** | Open since July 1, 2026 | A foreign dental degree equal to at least an Indiana DDS or DMD, as the board determines; transcripts with English translation; a credential verification (a NACES member or board-approved organization); national board scores; a clinical exam from CDCA-WREB-CITA, CRDTS, or SRTA; current BLS or ACLS; no more than three exam attempts; $100 application fee (IC 25-13-1-6, as amended by HEA 1254; Indiana PLA) |
| **Virginia** | Law in force; **no licenses yet** | Graduate of a dental school outside the US who was or is eligible to practice dentistry outside the US, plus the JCNDE hygiene exam and a clinical exam acceptable to the Board (Va. Code § 54.1-2722). The Board said on July 21, 2026 that regulations must come first, estimated for late winter or early spring 2027 |
| **Connecticut** | Open, with training | A foreign DDS or DMD, the written and practical exams, **and** enrollment in a Connecticut CODA-accredited hygiene program with at least one year of clinical training in an affiliated community health center (Conn. Gen. Stat. § 20-126i(b)) |
| North Carolina | Not allowed | The Board: foreign-trained dentists "MAY NOT work as dental hygienists in NC but may work as a Dental Assistant II" |
| Georgia | Not allowed | Requires a CODA-accredited hygiene program |
| Texas | Not allowed | Requires a CODA-accredited school of dentistry or dental hygiene, or an approved alternative hygiene program |
| California | Not allowed | AB 1952 would have created a track; the California Dental Association reports it "will not advance in 2026" |
| Oregon | Not for dentists | Oregon's foreign route covers graduates of foreign hygiene programs, not dentists |

Rules are moving quickly. Check the state board's current page before you pay for exams.

## The Florida route, step by step

Florida sets the route out in statute (s. 466.007) and Board rule:

1. **Pass a national written exam first:** the NBDHE **or** the National Board Dental Examination (s. 466.007(2)(c)). Proof of the pass goes into your credentials package. If you already passed a national dental board exam, confirm with the Board whether it counts before booking the NBDHE.
2. **Complete an HIV/AIDS course** of the scope the Board defines (s. 466.007(3)(b)).
3. **Get your credentials reviewed.** File the Board's Application for Dental Hygiene Credentials Review for Graduates from Non-Accredited Dental Colleges or Schools (form DH-MQA 1253) with proof of 4 academic years of dental education, including an **ECE year-by-year evaluation**, a diploma comparable to a DDS or DMD, and your national board result. You apply for the remaining exams only after the Board approves your credentials (rule 64B5-2.0144).
4. **Pass the ADEX dental hygiene exam** through CDCA, with 75 points or more on both portions. International graduates need a state board authorization letter to register, valid up to 18 months, and every part must be finished within 18 months of the first attempt.
5. **Pass the Florida laws and rules exam** with 75% or more. [Florida Dental Hygiene Laws & Rules prep](https://dentovio.com/products/florida-dental-hygiene-laws-rules/index.html.md) covers that exam.

**The trap after a first failure.** If a graduate of a nonaccredited dental school fails the clinical exam once, Florida requires a **remedial dental hygiene course of at least 20 hours of live-patient clinical instruction** before the retake. After three failed attempts, you must complete one academic semester of senior-level clinical coursework at a hygiene school (rule 64B5-2.0135). Prepare to pass the clinical exam the first time.

## The NBDHE and the ADEX hygiene exam

**NBDHE (National Board Dental Hygiene Examination), 2026 Candidate Guide**

- **Eligibility:** "A dentist is eligible for examination if the eligibility requirements for the INBDE are met." For a non-CODA graduate, that means ECE confirmation of your dental degree and a DENTPIN, as for the INBDE ([how that works](https://dentovio.com/inbde/for-international-dentists/index.html.md#ece)).
- **Format:** 350 multiple-choice items: 200 discipline-based and 150 case-based across 12 to 15 patient cases, in 7 hours 30 minutes of testing within a 9-hour day.
- **Passing:** a scale score of 75 (scale 49 to 99). Results usually arrive within three weeks.
- **Retakes:** at least 60 days apart, no more than four attempts in a year, and a pass within five years or five attempts; after that, one attempt every six months.
- **Fees:** $600 exam fee. JCNDE lists a $435 international processing fee for candidates from non-CODA hygiene programs; confirm with JCNDE whether it applies to you.
- JCNDE expects updated NBDHE test specifications in November 2026.

Preparing for it: [NBDHE exam prep](https://dentovio.com/products/nbdhe/index.html.md).

**ADEX Dental Hygiene Examination (CDCA-WREB-CITA), 2026**

- A computer-based simulated clinical exam (CSCE OSCE) and a simulated patient treatment clinical exam (SPTCE).
- **Fee:** $1,150, plus a facility fee that varies by site and a $150 typodont fee. A second clinical attempt carries no exam fee, a third costs $795, and a CSCE OSCE retake costs $275.
- **International graduates** of non-accredited dental schools must submit a **state board authorization letter** naming the exam; it is valid up to 18 months. Apply through the state board.
- All parts must be passed within 18 months of the first attempt; three failures of the same part fail the whole series; there is a 10-day wait after a failure.

**Budget for the Florida route:** NBDHE $600, ADEX $1,150 plus $150 and the facility fee, an ECE evaluation, the HIV/AIDS course, and Board application and exam fees (Florida's statute caps them at $100 and $225).

## Working as a dental assistant while you pursue licensure

In states without a hygiene route, dental assisting is often the fastest way back into a US practice. It also builds the US clinical experience that advanced-standing programs weigh.

- **DANB CDA, Pathway III:** DANB's Certified Dental Assistant exam accepts "graduation from a dental degree program outside the U.S. or Canada," plus current CPR, BLS, or ACLS. The CDA exam has 245 questions in 195 minutes. Prep: [DANB CDA exam prep](https://dentovio.com/products/danb-cda/index.html.md).
- **State rules decide your duties.** Assistant registration and allowed duties are state-specific. North Carolina, for example, lets foreign-trained dentists work as a Dental Assistant II.
- **Pay:** the BLS median wage for dental assistants was $48,070 in May 2025.

## Frequently asked questions

### Can a foreign-trained dentist work as a dental hygienist in the US?

Yes, in a few states. Florida and Indiana accept a foreign dental degree for the hygiene license with credential review and exams; Connecticut adds a year of clinical training through a state hygiene program; Virginia's law is in force but licenses wait for regulations, estimated for early 2027. Most other states require a CODA-accredited hygiene program.

### What exams does a foreign dentist need for a Florida hygiene license?

Pass the NBDHE (or show a passed National Board Dental Examination) and submit it with your credentials application. After the Board approves your credentials, pass the ADEX dental hygiene exam through CDCA (75 or more on both portions) and the Florida laws and rules exam (75% or more).

### Can foreign dentists take the NBDHE?

Yes. JCNDE's 2026 guide says a dentist, from an accredited or nonaccredited program, is eligible if they meet the INBDE eligibility requirements, which for a non-CODA graduate means ECE confirmation of the degree.

### How much does a dental hygienist make in the US?

The BLS median annual wage for dental hygienists was **$98,100** in May 2025, and employment is projected to grow 8% from 2025 to 2035.

### Can foreign-trained dentists become hygienists in Virginia now?

Not yet. The law took effect July 1, 2026, but the Virginia Board of Dentistry says regulations must be adopted before any license can issue, which it estimates for late winter or early spring 2027.

### Can a foreign dentist become a certified dental assistant?

Yes. DANB's CDA Pathway III accepts graduation from a dental degree program outside the US or Canada, plus current CPR, BLS, or ACLS. Your state still sets what an assistant may do.
